UnitedHealthcare broadens zero-premium Medicare Advantage options
UnitedHealthcare is expanding its focus on low-cost Medicare Advantage plans for 2027, with nearly all eligible consumers in its service areas able to choose a $0-premium option. The insurer announced the shift ahead of the Medicare open enrollment period, which begins October 15.
The company’s 2027 Medicare Advantage plans will cover 90% of eligible consumers with access to a $0-premium choice, while over 94% of Medicare-eligible people nationwide will have access to at least one UnitedHealthcare Medicare plan.
Most plans will also include $0 primary-care visits, preventive services, and Tier 1 prescription drugs, though specific benefits vary by location and plan type.
UnitedHealthcare President Bobby Hunter emphasized the value of Medicare Advantage in a statement, calling it a solution designed to meet the real needs of beneficiaries.
The insurer’s HMO plans—accounting for 95% of its Medicare Advantage offerings—will remain premium-free for most enrollees, while members in these plans spent an average of $175 less annually out of pocket compared to those in its local preferred provider organization plans, according to the company’s 2025 claims data.
Special-needs plans will also see broader availability. The insurer’s dual-eligible plans, targeting those qualifying for both Medicare and Medicaid, will reach about 74% of eligible individuals. Chronic-condition special-needs plans, designed for conditions like diabetes and heart failure, will be available to roughly 48 million Medicare-eligible consumers.
UnitedHealthcare is also updating its member identification system. Starting in 2027, the company will separate its Medicare Advantage ID card from the UCard, which currently handles supplemental benefits, credits, and rewards.

Dual-eligible members will gain the ability to pay participating utility providers directly through Mastercard’s network. The UCard will remain usable at over 70,000 retail locations, including major chains like Walmart and Target, while members will continue to access more than 60,000 network pharmacies and 100-day medication supplies for many prescriptions.
The move comes as Medicare Advantage premiums are projected to decline nationally. The weighted average monthly premium for these plans is expected to drop by 16.5%, from $14.37 in 2026 to $12 in 2027. For plans that include prescription-drug coverage, the average monthly Part D premium is projected to fall by 38%, from $11.32 to $7.
CMS Administrator Dr. Mehmet Oz noted the agency’s efforts to keep high-quality care affordable, as Medicare Advantage enrollment is projected to reach 34 million in 2027, nearly 47.4% of all Medicare beneficiaries. Over 99% of beneficiaries will have access to at least one Medicare Advantage plan, with 97% facing at least ten choices.
UnitedHealthcare’s push toward $0-premium plans aligns with broader industry trends. While the company reports strong financial performance, second-quarter net income rose 61% year-over-year, other insurers are scaling back. Humana is exiting markets affecting 600,000 members, and Providence Health Plan is winding down its Medicare Advantage operations, impacting over 64,000 enrollees. These shifts suggest a competitive environment where cost and accessibility remain key differentiators.
